*The 2021 Flu Vaccine website (for scheduling an appointment) & COVID-19 Vaccine website (for consent form and fact sheet) can only be accessed on the EB Network; sites are located internally on Homeport for employee access only.
As we continue to face the COVID-19 pandemic, timely seasonal flu vaccination is a priority this year. If you haven’t received your flu vaccine, there’s still time to do so. There are remaining flu vaccine clinics at Connecticut EB locations for employees; both the regular and high-dose (age 65+) flu vaccines are available.
COVID-19 vaccines are also being offered at the flu vaccine clinics. All three brands of the COVID-19 vaccine – Pfizer, Moderna and Janssen (J&J) – are now available! When you arrive for your flu vaccine, simply ask to receive the COVID-19 vaccine if you wish to do so. If you attend a clinic to receive your second dose, please bring your vaccination card with you. The COVID-19 Vaccination Consent Form and Vaccine Fact Sheet will be available at the vaccination clinics and can also be found on our COVID-19 Vaccine website: https://homeport.ecms.gdeb.com/medical/COVIDVaccine/SitePages/Home.aspx.
*Flu vaccines can be administered simultaneously with or any time before or after the administration of a COVID-19 vaccine.
Remaining clinic dates, times and locations can be found on the 2021 Flu Clinic website https://homeport.ecms.gdeb.com/medical/flushots/SitePages/Home.aspx by clicking on each EB site icon; this information is also listed on the Homeport calendar. For your convenience, all remaining flu/COVID vaccine clinics have also been listed at the end of this notice.
If you have any questions, please contact EB Wellness at ebwellness@gdeb.com or call the Groton Yard Hospital at (860) 433-3470.
