The federal government recently released new COVID workplace safety protocols, including a COVID vaccination mandate, for federal contractors. Our company has asked for clarity around this mandate, and we will continue to update you as we learn more.
In the meantime, we ask that vaccinated employees please submit proof of vaccination status, even if you have previously reported your status to our Medical Team, by sending an email to proofofcovidvaccination@gdeb.com. Please include your name and badge number in the subject line of the email and attach proof of vaccination, either a photo taken with your personal phone or scanned documentation.
EB policy states that employees cannot take any photos on company property. Please take photos of documentation offsite. If you do not have direct access to a copy machine or scanner, please ask your supervisor for the one close to your work location. Please do not send your documentation via interoffice or regular mail.
Proof of vaccination can be one of the following:
- A copy of the record of immunization from a health care provider or pharmacy
- A copy of the COVID-19 Vaccination Record Card
- A copy of medical records documenting the vaccination
- A copy of immunization records from a public health or state immunization information system
- A copy of any other official documentation containing the following data points:
- Type of vaccine administered
- Date(s) of administration
- Name of the health care professional(s) or clinic site(s) administering the vaccine(s)
By providing this documentation, employees are certifying the information they are submitting is true and correct. Please note that your information is treated as protected personal information by the EB Medical staff.
If you’ve been vaccinated but have lost your card, you can contact the vaccination provider site where you received your vaccine. Your provider should be able to give you a new card or documentation about the vaccine you received. If the location where you received your vaccine is no longer operating, you should contact your State or local health department’s immunization information system (IIS) for assistance. You can also contact your State or local health department if you have additional questions about vaccination cards or vaccination records.

COVID-19 vaccines are available at all EB clinics and pharmacies for employees, as well as in pharmacies and clinics in the communities surrounding our facilities. Dependents of EB employees can receive a vaccination at the EB Family Wellness Center in Groton or at a community location.
